AGREEMENT and the CREDIT CARD AUTHORIZATION form, then
fax both forms to Pet Air Carrier, LLC at 770-645-1863.
The Transportation Safty Administration requires Pet Air
Carrier, LLC, an Indirect Air Carrier, to have a SIGNED
SHIPPING AGREEMENT in our office a MINIMUM of 7 days prior
to shipment.
